Facial aging is caused by a thinning of boney facial structure (osteopenia) and shrinkage of natural subcutaneous fat and deep fat pads which support skin. This loss of volume and structural support alters a youthful smooth “V” shaped face with high cheekbones and attention up to the eyes towards an “A” shaped face with hollowing of temples and cheekbones and fullness in the lower face. This results in areas of fullness separated by hollows or shadows.

To accomplish this, she asks patients to provide an old photo so she can create treatment to help re-create your younger self. Then, she often breaks up the treatment into two sessions.
This allows her to wait until the result of the first session (after any residual swelling has resolved) before adding more filler, if necessary, avoiding the potential for adding too much.

Some patients opt to have facial fat grafting, which is a procedure that extracts fat from the thighs or belly and injects it into the face to replace lost volume. In some cases, a combination of facial fat grafting and dermal fillers may be advantageous. Botox® is often used along with dermal fillers to correct the lines and wrinkles caused by muscle activity rather than volume loss.

CANDIDATES FOR DERMAL FILLERS
Dermal fillers can improve many different signs of aging:

During your consultation and thorough examination, Dr. Kashyap will evaluate you to see if you are a good candidate for treatment. Dermal fillers are minimally invasive, making them a safe and effective option for most healthy people. However, you should let Dr. Kashyap know if you have a history of cold sores. During the consultation, she will also recommend which fillers would work best for you.
The primary consideration is whether fillers will provide you with the results you want.
In cases of severe signs of aging, particularly around the eyes, Dr. Kashyap may suggest a combination of dermal filler, Botox® and chemical peels. In some cases she may suggest a lower eyelid blepharoplasty.
Prior to your treatment, Dr. Kashyap will ask you to avoid aspirin and ibuprofen for two weeks before your appointment. She will also ask you to take oral Arnica and Bromelain three days before the treatment. These preparations will help to prevent bruising from the injections.
TYPES OF DERMAL FILLERS
Dr. Kashyap offers a variety of dermal fillers, each of which has its own specific properties. Most fillers last at least six to nine months, although some last longer. The duration can increase after each treatment, up to 18 months in some cases.
This family of fillers uses hyaluronic acid, a complex of sugar molecules naturally found in the body. The Juvéderm® family includes Vollure, Volbella, Voluma, Ultra, and Ultra Plus. Each works in a slightly different way. Some are better for plumping the lips, while others are thicker and perfect for the deeper creases such as nasolabial folds. The thicker formulas are even capable of filling hollowed cheeks and lifting sagging jowls. Most Juvéderm® formulas last 12-18 months, although this varies depending upon the patient.
Restylane® is a clear gel hyaluronic acid that is used to plump the lips (Restalyne Silk), contour cheeks and jawline (Restalyne Lyft®), smooth transitions between different part of the face: temples-cheeks; chin-jawline; cheeks- lips/nose; chin-lips. Since the area around the mouth is so dynamic, facial aging is often recognized by marionette lines, nasolabial folds, and jowls. Restalyne Refyne and Restalyne Defyne are formulated for injections in these areas to allow dynamic movement while maintaining support. These formulas last up 12 months.
This is another Hyaluronic acid filler that has a particularly smooth consistency, making it best for superficial fine lines. It is often used on vertical lip lines, for example, for a subtle effect. Belotero along with Restylane® Silk and Volbella are used for micro-infusions of hyaluronic acid into the superficial skin layers to hydrate the skin and promote collagen production. Belotero lasts four to six months.
This is another hyaluronic acid filler is formulated to be smooth and target shallow facial lines. It is often used on vertical lip lines, for example, often referred to as lipstick lines. Belotero lasts four to six months.
Before & After
DERMAL FILLER PROCEDURE
Dermal Fillers are non-surgical and are administered using a fine needle. Your treatment will take about 30 minutes.
We apply a topical numbing cream or an ice pack to the treatment areas before the injections. The dermal filler formulas also contain Lidocaine, which provides additional numbing during the treatment.
Dr. Kashyap will administer the injections in the designated areas. You can go home immediately after your procedure.
RECOVERING FROM DERMAL FILLERS
No downtime or recovery is needed after your appointment. Some patients do experience some mild bruising, redness, swelling, and discomfort in the injection areas, but these symptoms resolve within a few days. Cold compresses and Tylenol can help if you experience discomfort or swelling. To alleviate these symptoms, patients can also take Arnica and Bromelain before and after their treatment. We recommend that you avoid exercise and alcohol intake for 48 hours.
Results are immediate from most dermal fillers, but if you have swelling, you may not see the final result for about a week.
